FORMER EMPLOYEES
As a former employee, you may have questions regarding a variety of topics. The following is intended to provide detailed information regarding the most common questions that former employees may have.
Q: I had money in a 401(k) and I want to get it back. What do I do?
A: Call Prudential 1-877-778-2100
Q: What happens to my health insurance coverage?
A: The Company will continue whatever health insurance benefits you were receiving
through the remainder of the month in which your employment terminated.
Q: How can I continue my health insurance coverage?
A: The Termination of Benefits letter you received explained your option to continue
your health insurance benefits under COBRA for a period of 18 months. Should you
elect COBRA coverage, CEC will provide a premium credit equal to the number of weeks
of severance you are entitled to. Thereafter, you will be solely responsible for
all premiums effective the first day of the month following your termination.
Q: How soon can I expect COBRA information?
A: You should receive a letter from Ceridian within the next two weeks of your date
of termination if you participated in CEC’s health benefits. Remember that this
change in your employment status is considered a qualifying event and if your spouse
has medical insurance, you have the option of changing your spouse’s coverage to
include you. If you have questions, call Ceridian at 1-800-877-7994.
Q: I am currently enrolled in school and have been receiving tuition reimbursement.
Will I still receive reimbursement?
A: You must submit receipts, grades and other appropriate documentation in accordance
with the policy and you will be reimbursed according to the program guidelines.
You will be allowed to complete the current class under the terms of your current
waiver program.
Q: What if I still have business expenses to submit?
A: You can mail your completed expense reimbursement form, along with all receipts,
to:
CEC Corporate – Kathy Lothary, Accounts Payable, 2895 Greenspoint Parkway, St. 600, Hoffman Estates, IL 60169
